1. Services

Labs and Health of Miami LLC ("Labs & Health") provides mobile phlebotomy and specimen collection services only. We are NOT a medical provider, physician, laboratory, or diagnostic facility. We do not diagnose, treat, or provide medical advice. All laboratory testing is performed by certified third-party laboratories. Always consult a qualified healthcare provider for medical advice.

2. Payment Terms

All services are self-pay. We do not bill insurance, Medicare, or Medicaid. Payment is required at or before the time of service. We accept major credit cards and HSA/FSA cards. You may submit your receipt to your insurance company for potential reimbursement. Final pricing may vary based on distance, scheduling, and special handling needs.

3. Cancellation Policy

You may cancel or reschedule your appointment at no charge if you provide at least 24 hours notice before your scheduled appointment time. Cancellations made less than 24 hours before the appointment may be subject to a cancellation fee.

4. Purchase & Refund Policy

  • All purchases are final. Once a lab panel kit has been ordered from the laboratory, the order cannot be cancelled or refunded.
  • If you need to change your appointment date or time, you may reschedule at no additional cost by contacting us at least 24 hours before your scheduled appointment.
  • If you are unable to attend your appointment and do not reschedule, the purchase amount will be applied as account credit for a future visit.
  • No-shows without prior notice will not be eligible for rescheduling or credit.
  • Labs & Health reserves the right to cancel or reschedule appointments due to weather, emergencies, or staff availability. In such cases, you will be notified and offered a new appointment or full credit.

5. Specimen Collection

Our mobile phlebotomy services follow OSHA safety standards and HIPAA-compliant practices for handling all protected health information. Specimen collection is performed in accordance with CLIA guidelines and laboratory protocols required by the receiving certified laboratory. Our phlebotomists are licensed and certified in the state of Florida. All patient data is handled in accordance with HIPAA privacy and security requirements.

6. Results

Laboratory results are processed and delivered by the certified third-party laboratory, not by Labs & Health. Results are typically available within 24-48 hours but processing times may vary. Labs & Health is not responsible for the interpretation of laboratory results. Please consult your healthcare provider for result interpretation.

7. Limitation of Liability

Labs & Health provides specimen collection services with reasonable care and professionalism. We are not liable for laboratory errors, result delays, or medical decisions made based on test results. Our liability is limited to the amount paid for the specific service in question. We are not responsible for adverse reactions to blood draws, including but not limited to bruising, fainting, or infection, though we take all reasonable precautions to minimize such risks.

8. Privacy

Your privacy is important to us. Please review our Privacy Policy for information about how we collect, use, and protect your personal information. We follow HIPAA-compliant practices in handling all patient information and protected health information (PHI). We implement administrative, physical, and technical safeguards to protect your data.

9. Website Use

The information on this website, including our quiz and health guides, is for informational and educational purposes only. It does not constitute medical advice, diagnosis, or treatment. The quiz provides panel recommendations based on general health goals and is not a substitute for professional medical guidance.

10. Governing Law

These Terms & Conditions are governed by the laws of the State of Florida. Any disputes arising from these terms or our services shall be resolved in the courts of Florida.
